VolunteerApplication Process Please log onto the eAYSO Web site, Please bring 2 copies and a valid photo ID with you to registration.complete the volunteer information, and print and sign two copies of the forms. Forms MUST be turned in in person to one of the Child and Volunteer Protection Advocates (CVPA's) during open registration. offline pdf forms: Volunteer Application Form / Volunteer Application Form Youth All persons who work with the players in any way must complete and submit the registration forms and complete Safe Haven training before working with the players.
